Update US subchannels

The main goal of this update is to correct the names of US subchannels and bring their descriptions more in line with other well-known databases.

The new naming scheme includes official station call sign and full subchannel id:

K09QP-D2 => K09QP-D 9.2
KPMF-LD4 => KPMF-LD 26.4
WPXP-DT6 => WPXP-TV 67.6
WQCW-DT2 => WQCW 30.2

This should make it easier to identify missing subchannels and keep their descriptions up to date.

Changes:

• channels.csv: updated id, name, alt_names, network, owners fields
• feeds.csv: updated id, name, broadcast_area, format fields
• logos.csv: updated channel, feed fields
• blocklist.csv: updated channel field

The update applies only to the channels listed on fcc.gov and RabbitEars.info.

ToDo:

• iptv-org/iptv: replace tvg-id (Update tvg-ids for US subchannels iptv#35368)
• iptv-org/epg: replace xmltv-id (Update xmltv-id for US subchannels epg#3051)
